February 8, 2010 Speech Language Pathologist - Hilo (F/T) This position provides speech language therapy services and individualized treatment plans, applying principles and practices to children with special needs, promoting optimum speech language development utilizing a transdisciplinary approach. Home visits and visits to other community program sites are required to provide services. .5 full-time equivalent (FTE) with EI working with children ages birth to three years and .5 FTE with FISH working children ages 3 to 6 years. Experience with infants preferred. CFY candidates are accepted in select locations. Appropriate degree and license required. Physical Therapist - Kailua (F/T) & Honolulu (F/T & P/T) This position provides physical therapy services and implements individualized treatment plans for program participants, including written reports and evaluations. Masters or Doctorate degree and license to provide physical therapy in Hawaii required, experience in pediatrics or children preferred. Social Worker - Kailua (F/T) & Kapolei (F/T) This position provides immediate interim and on-going culturally competent, family-centered care coordination services for infants and toddlers with special needs and their family. Care coordination services include supporting the child through intake, evaluation, developing an Individualized Family Support Plan (IFSP), and transition from early intervention. Experience with infants/pediatrics preferred. Appropriate degree and license required. Youth Services Special Projects Consultant - Honolulu (Short-term, approx. 3-6 months, not to exceed 9 months) This position performs an extensive statewide needs assessment, evaluation, and comprehensive action report of the programmatic needs, related costs, staffing requirements, and long-term funding opportunities for programs & services for children ages 5 20 with disabilities or special needs & their families. To include extensive fact-finding and interviews with key stakeholders in private, public and non-profit sectors of the community. Key deliverables include recommendations of a sustainable programmatic model and long term funding sources. Experience in the areas of social services, services for children with special needs & their families, youth development, or special education preferred. Current First Aid and CPR certification required.
Activities Leader - Honolulu (P/T) This position plans, coordinates and supervises activities and personal care for youth with developmental disabilities in an after school and school break program. HS diploma or equivalent. 2 years experience with developmental disabilities population. Background in special education preferred. Current First Aid and CPR certification and TB clearance required.
Program Aide - Honolulu (P/T) This position assists with implementation of age-appropriate activities for youth with developmental disabilities in an after school and school break program. Administrative Home & Community Based Services Seeking motivated individual with proven human services background to provide leadership and facility, staff and managerial oversight of adult day programs in Ewa Beach and Makakilo. Responsible for the overall administration, operation & management of the program, including staff, coordination of direct services, program planning, contractual compliance & budgeting/billing. BS degree in a field of human services required. Experience working with developmental disabilities population preferred with 2 years managerial experience. Current First Aid and CPR certification and TB clearance required. This position assists teens & adults with developmental disabilities 1 on 1 in various community settings.
